Telehealth Terms of Service

These Terms of Service ('Terms') govern your access to and use of Preventive Diagnostic Group’s ('PDG') telehealth services, platforms, and related offerings (collectively, the 'Services'). By using our Services, you agree to these Terms and acknowledge our commitment to HIPAA compliance and patient privacy. 

PDG complies with the Health Insurance Portability and Accountability Act (HIPAA) and all applicable regulations. We maintain administrative, technical, and physical safeguards to protect Protected Health Information (PHI) against unauthorized access, disclosure, alteration, or destruction.

PDG provides secure telehealth technology that connects patients with independent licensed healthcare providers. PDG is not a medical provider and does not practice medicine. Providers using our platforms are solely responsible for patient care decisions, diagnoses, prescriptions, and treatment outcomes.

By accessing PDG’s Services, you consent to receive medical consultations via telecommunication technology. You acknowledge understanding the benefits and potential limitations of telehealth and consent to the collection, use, and storage of your medical information for healthcare delivery and compliance purposes.

You agree to provide accurate and complete information and use PDG’s Services only for lawful purposes. You are responsible for maintaining the confidentiality of your login credentials and for any activity conducted under your account. Misuse of the platform may result in termination of access.

PDG and its affiliates are not liable for indirect, incidental, or consequential damages arising from the use or inability to use the Services. PDG disclaims responsibility for the actions or omissions of independent providers.

These Terms are governed by the laws of the State of Illinois. Any disputes arising from or related to PDG’s Services shall be brought exclusively in the state or federal courts located in Illinois.
